Students who qualify for second semester rotations (see the "Student Responsibility Form" in
the Application Packet) will have an opportunity to visit several Samaritan Health Service
and Albany community sites. The following is a sampling of sites available (click on the site names below to see pictures):
Vaccinations:
The shots you are required to have are listed below. It is possible to decline shots (parents
must sign "Shot Declination Form") but you will not be allowed to visit certain sites. Shots
required:

Negative TB test. To be completed after September 1 of the year you are in the class. The
test must be read and signed by the person reading your results. The date read, mm induration,
and the signature of the person reading the result must be written on the Samaritan Health Services
Vaccination Form.

Record of two Measles, Mumps, Rubella (MMR) shots. One before entering school and a booster
given later.

Hepatitis B (It is a series and you will need to have at least started the series by the end
of the 1st 6 weeks of school).

Chicken Pox. You must have the approximate date you had Chicken Pox or you must get the vaccine.

You need to show proof of these shots by the end of the first 6 weeks. Mrs. Lemerande needs a copy
(not the original) of your shot record in addition to the Samaritan Health Services form filled out.
Do not get needed shots or proof until you know you have been accepted into the class. Also, you must
be accepted into 2nd Semester Rotations. Getting these shots does not mean you are automatically
accepted. There are a limited number of sites for rotations so enrollment will be limited. However,
these shots protect you against disease and are recommended by many health care professionals.
